Applying for Affordable Housing and Related Forms

These forms can be viewed and saved to your computer, printed, or picked up in person at several locations in Niagara region.

  • Request for Special Priority Status Form

    Fact Sheet for applying for Special Priority Status.

    If you have been abused by someone you live with, or have lived with, you may be eligible for Special Priority Status. You will need to provide documentation from a designated professional.

  • Request for Urgent Status Form

    Fact Sheet for applying for Urgent Status.

    If you have physical limitations and your health is significantly compromised by where you live, or you are experiencing extreme hardship because of where you live, you may be eligible for Urgent Status. You will need to provide documentation from a designated professional.

  • Request for Homeless Status Form

    If you are currently living without shelter, you may be eligible for Homeless Status. You will need to provide documentation from a designated professional.

  • Request for an Additional Bedroom Form

    For Medical Reasons or a spouse or same-sex partner requires a separate bedroom because of a disability or medical condition

  • Request for an Accessible Unit Form

    There are a number of affordable housing units that have been modified with accessibility features for people with physical disabilities. These modifications may include wheelchair/scooter accessibility; design and shape of rooms and the size of doorways.

    A household may be eligible for accessible housing if one or more household member requires accessibility modifications in order to live independently.

  • Request for Review of Decision - Appeal

    After a Notice of Decision is given to an applicant or rent-geared-to-income tenant, they have a right to request a review of the decision within 10 business days of receiving the Notice.

    The request for review must be received in writing and explain why you disagree with the decision and include supporting information.

  • Transfer From Market to Rent Geared-to-Income: Request for Rent Geared-to-Income Subsidy Form

    If you are currently a tenant/member and are paying market rent, you may be eligible to apply for a subsidy.
