Applying for Affordable Housing and Related Forms

These forms can be viewed and saved to your computer, printed, or picked up in person at several locations in Niagara region.

• Request for an Additional Bedroom Form
  
  For Medical Reasons or a spouse or same-sex partner requires a separate bedroom because of a disability or medical condition
  
  
• Request for Health and Safety Priority Status Form

  If you feel your personal safety is significantly at risk or you have a serious health condition or terminal illness, you may be eligible for Health and Priority status. You will need to provide documentation from a designated professional.

  If you are currently living without shelter, you may be eligible for Homeless status. You will need to provide documentation from a designated professional.

  If you have been abused by someone you live with, or have lived with, you may be eligible for Special Priority status. You will need to provide documentation from a designated professional.
  
  
• Request for Modified Unit Form
  
  If you have significant mobility issues and rely on a wheelchair as your main source of mobility, you may be eligible for a modified unit.

  If you are currently a tenant of Niagara Regional Housing and would like to move to a different Niagara Regional Housing unit, complete the transfer and building selection forms:

• Transfer From Market to Rent Geared-to-Income: Request for Rent Geared-to-Income Subsidy Form
  
  If you are currently a tenant/member and are paying market rent, you may be eligible to apply for a subsidy.
